What you’ll need to claim a profile

A verified email address, normally on the same domain as your organization’s official website.
Your name, role, and authority to manage the requested organization or locations.
Approval by an Injury Med Map administrator before any management access is granted.

Use a different email domain? You can still request access. We’ll manually verify your authority, for example by contacting the organization using independently confirmed details. A matching domain alone does not grant access.

After approval, you can manage public contact details and request updates for your approved locations. Claiming a profile does not confirm acceptance of PI or workers’ comp cases.

Do not send passwords, identity documents, or patient information with your claim.

Please share basic contact information only. Do not include patient identifiers, medical records, or detailed injury information.
