A callback request starts a conversation
Choose “Request a scheduling callback” when you want the intake team to follow up about availability. The request does not reserve a time or book an appointment. The provider will need to confirm the next steps directly.
Choose a response method
Select email, phone, or either. If you request a phone response, provide a contact phone number. Check the reply email and use Change if the response should go to a different address for this request.
Give a useful contact window
A preferred contact window helps explain when you would like to be reached. It is not a confirmed callback time. The form describes this window in the clinic’s local time.
Identify who is completing the form
Patients can submit a request themselves. Attorneys, case managers, paralegals and other law-firm staff should select their role and include their firm and contact phone. Family members, friends and support people have their own option; including the law firm when known can help the office coordinate intake.
Keep the message brief
Include only the basic information the intake team needs to respond. Do not include patient identifiers, detailed medical histories or records. Ask the provider for its secure intake process when those materials are needed.
Confirm the arrangements with the office
A directory message does not confirm service availability, case acceptance, payment terms or an appointment. Ask the office to clarify those details before relying on a proposed arrangement.
